Activities Included:

• Ballroom Dance

• Latin Dance

• Shine Bright

• Zumba

• Other Physical Activities (e.g., Yoga, Pilates, Hip Hop Dance, etc.)

Agreement and Release of Liability

1. Assumption of Risk

   I acknowledge that participation in dance classes, rhythmic gymnastics, ballet, and other physical activities involves risks and dangers of bodily injury. I understand these risks and voluntarily assume all risks associated with participating in any activity at KwanHub Dance Studio.

2. Waiver and Release

   I, for myself, my heirs, executors, administrators, and assigns, hereby waive, release, and discharge KwanHub Dance Studio, its instructors, employees, owners, agents, and representatives from any and all claims for damages for death, personal injury, or property damage which I may have, or which may hereafter accrue to me, as a result of participation in activities at KwanHub Dance Studio.

3. Medical Release

   I declare that I am physically fit and have no medical condition that would prevent my full participation in these activities. In case of a medical emergency, I authorize KwanHub Dance Studio staff to provide or seek medical assistance on my behalf, and I take full responsibility for any medical expenses incurred as a result.

4. Photography and Media Release

   I consent to the use of photographs, videos, and/or recordings of me for advertising, promotional, or related purposes by KwanHub Dance Studio, without compensation or prior notice.

5. Understanding of Agreement

   I have read this agreement, fully understand its terms, and understand that I am giving up substantial rights by signing it. I sign this agreement freely and voluntarily, without any inducement.
